Consent Release Requests
CONSENT RELEASE REQUEST LISTS (Requests for Client Information)
When a CMBHS provider receives a request for client information from another CMBHS provider, it is displayed on the receiving provider’s Consent Release Requests Administrative Action List,
From the Consent Release Requests Administrative Action List, the provider can view a Request for Release of Client Information sent to them by another provider and decide whether or not to release the information. They can take the action and then delete the Request from the list when the needed action has been taken.

Before You Start
- All business entities using CMBHS can document and receive requests for Release of Client Information.
- A CMBHS user must be assigned a role that permits viewing and/or documenting in the Consent Release Requests, Administrative Action List.
- The Consent for Disclosure of Information driving the request must::
- have all fields completed
- be signed by the client or the LAR
- be saved in Closed Complete status.
Business Rules
- CMBHS filters the requests so that only requests for matching service information types display at each location.
- The Consent Release Requests Administrative Action List operates at the business entities’ parent and service Locations.
- The business entities’ parent Location can see all the Requests that come to the agency and can take action on them.
- The business entities’ service Locations can only see the Requests for Locations that are of the same type as theirs. For example, at a provider’s substance abuse service Locations, only requests for substance abuse service clients will display on their list. At an OSAR Location, only requests to release OSAR client records will display.
- The Requests disappear from the Action List when the action takes place to release the records or deny the release of records and when the release expires.
- Once the Release button is hit, it is disabled so that user cannot re-send the Request again.
- Pressing the Release button causes the documents to be sent to the disclosee and information about the disclosure is placed in the footer of the Consent form.

How to Access Consent Release Requests
- The Consent Release Requests Administrative Action List is located on the Administrative toolbar that is at the top of every page of CMBHS.
- Hover over Provider Tools, then SCROLL down to Administrative Action Lists and then CLICK on Consent Release Requests. Page CRR048 will display.
How to Respond to a Request for Release of Client Records
- To View a Requesting Entity’s request for release a client’s records to the, CLICK in the row with the client’s name to highlight it and then CLICK on the View button.
- The request, page CRR049 submitted by the Requesting Entity will display along with 5 buttons in the upper right hand corner and at the bottom of the page:
- Print – CLICK on this button if you would like to print a copy of the Request submitted by the Requesting Entity. After CLICKING on Print you will see a pop-up box asking you if you if you would like to Open or Save. Select Save and Save the document to an appropriate place for client health care information. Then print the document the way you would any other document in pdf format.
- Attachments – CLICK this button if you would like to view any attachments that may have been included by the Requesting Entity. Page ATT090 will display. If any attachments were included they will display on the list. CLICK in the row of the attachment you would like to view.
- Release – CLICK on this button to release or give the entity the information they are requesting. The information will become available to the Requesting Entity almost immediately
- Deny Release – To deny the request and not allow the Requesting Entity to have the client documents
- Close – CLICKING on this button closes the page and will erase any data entered.
