In the General Tab, the user gathers information about why the client is presenting for services and documents that information in the General section. The data fields and questions under the General tab are the same for all clients receiving all service types and of any age.
Physical Health Tab – The user documents answers to medical questions specifically related to Substance Use Disorder symptoms and potential co-occuring medical conditions. These conditions may increase the severity of the consequences of substance use.
The Clinical Opiate Withdrawl Scale (COWS) and Clinical Institute Withdrawl Assessment of Alcohol Scale, Revised (CIWA-AR) display when the client is receiving services in a detoxification setting. The COWS will display only for clients with Opioid withdrawl diagnoses. The COWS and CIWA are available in the Blank Forms section of CMBHS separate from the Detoxification Assessment for use by contractors during detoxification services.
Two new assessment tools have been added to CMBHS for specific types of substance abuse. When the client is receiving services in a detoxification setting (a CMBHS Detoxification location) and has reported abusing alcohol, and alcohol has been entered in the substance of use table, the Clinical Institute Withdrawal Assessment of Alcohol Scale, Revised (CIWA-AR) displays for completion.
When the client is receiving services in a Detoxification setting and has reported abusing opioids, and an opioid is entered in the substance of use table, the Clinical Opiate Withdrawal Scale (COWS) displays for completion.
The CIWA displays only when the client is receiving services in a Detoxification setting and the client has an Alcohol Withdrawl diagnosis.
The CIWA is available in the Blank Forms section of CMBHS separate from the Detoxification Assessment for use by providers when CMBHS is not available.
All Questions – To complete the CIWA click on the radio button that best describes the client's symptoms.
Results – CMBHS will total all the scores and display the results.
If the client has indicated that they abuse an opioid and an opioid has been entered on the substance of abuse table on the General tab, the Clinical Opiate Withdrawal Scale (COWS) will display. The COWS is only available in the Detoxification Services assessment type.
The COWS is available in the section of CMBHS separate from the Detoxification Assessment for use by contractors.
Resting Pulse Rate – For the first question, Resting Pulse Rate, enter the beats/minute in the Beats/Minute text box and select one of the answers by clicking in the radio button.
All Other Questions – For all other items, CLICK IN the radio button that best describes the client's symptoms.
Results – CMBHS will total all the scores and display the results. The maximum number of points is 48. The number of points indicates the severity of the client's withdrawal.

In this section, clinicians document information about the client’s mental health when the client is not under the influence of mind-altering substances. During the interview, Clinicians need to ensure the client understands that the symptoms described on this tab represent things that happen when the client is not under the influence of mind altering substances. The information gathered is historical and current information.

The SUD Detoxification Service End Assessment has the same items displayed as the Initial SUD Detoxification Assessment.
CMBHS will carry forward answers to questions that do not require updated responses.
The clinician may edit any of the responses carried forward to the Discharge Assessment. These items should be edited if the client's information changes.
There are other items that must always be answered with new information from the client.
