Order of Mental Health Diagnosis on the
MH Diagnosis Page |
Background: In
June of 2018, changes were made to CMBHS to align it with the DSM 5. In
addition to conversion of the diagnoses descriptors from ICD-10 descriptors
to DSM 5 descriptors, the DSM 5 order of diagnoses rules, were also implemented.
The DSM 5 rules require that the principal diagnosis, the TRR Priority
Population mental health diagnosis, be in order 1. After
some months, it was noted that some batch files were being submitted without
having the principal mental health diagnosis in order 1. Due either to the
provider’s not having made the needed changes to their local system and/or
due to error. As a result, MH Assessments were calculating as LOC-9, when in
fact, the client did have a principal mental health diagnosis, it was just
not documented in the first order. At
the request of the LMHAs, a change was made to CMBHS to address the most
frequent type of error: a principal mental health diagnosis is documented for
the client but not in the first order. The Change: Consistent
with the DSM 5, HHSC MH Programs has not changed the program requirement that
the principal MH diagnosis be documented in order 1. This
change returns CMBHS back to how it functioned before the DSM 5 project was
deployed: when a user begins the process of documenting a MH Assessment
(CANS/ANSA) in CMBHS, CMBHS will search the client’s most recent diagnoses
for a qualifying MH diagnosis beginning with the first diagnosis and
searching through the next 5 for a total of 6. CMBHS will accept the first
qualifying MH diagnosis that it locates. When
diagnoses are batched, the diagnoses will be entered in the order submitted
and when a MH assessment is attempted, the search for a qualifying MH
diagnosis will go through the same process. The
first qualifying MH diagnosis will be used by the system to determine the
appropriate clinical scales for adults, and to calculate an LOC. This is how
the system worked prior to release of the DSM 5 change. Limitations: 1.
The
program requirement that the principal MH diagnosis be listed in order 1 has
not changed. The purpose of this change is to reduce the number of client’s
calculating to LOC-9 because of an error. 2.
CMBHS
will not differentiate between the principal diagnosis (the qualifying TRR
Priority Population diagnosis) and a qualifying MH diagnosis which means it
is a DSM 5 MH Diagnosis. 3.
If
the qualifying MH diagnosis is in order 7 – 18, it will not be located by
CMBHS and the Assessment calculator will find the client does not have a
qualifying MH diagnosis. |

Child and Adolescent Diagnosis and
Assessment |
The
error where CMBHS was accepting child and adolescent diagnosis that were not
mental health diagnoses as mental health diagnoses and allowing the
calculation of a level of care (LOC) has been corrected. The MH Assessment (CANS) will now only
calculate an LOC for a youth with a qualifying mental health diagnosis. |
